I'm pretty sure I missed some of the chaos mixes, since they were pretty faint on some of them, but I've almost completed the list for you.
Going by video cuts to find where new songs start may work most of the time, but sometimes the video editor will use two different vids for the same song -- the "Hajimete no Oto" song in this medley for example. Sometimes, if you flip through some of the alternate versions, you'll see that a Nicouser has written out the songs on the bottom of the screen in comments, or the videographer has embedded it somehow into the video. If you don't know Japanese, they at least help in discerning songs and catching some of the fainter chaos mixes.
